What You’ll Do- Coordinate patient care plans with physicians and multidisciplinary teams
- Support discharge planning and continuity of care services
- Advocate for patients and families while optimizing healthcare resources
- Monitor care progression, length of stay, and patient outcomes
- Strong communication, coordination, and critical thinking skills
