- Oversee the development and implementation of the physicians’ plan of treatment, ensuring services comply with physician orders and are received within regulatory time frames.
- Conduct regular case conferences/team meetings; keep staff informed; perform supervisory visits within organization and regulatory requirements.
- Match clients with staff based on needs, territories, client loads, and clinical skills.
- Participate in recruitment and orientation; assure adequate staffing and maintain vacancy and turnover metrics at established goals.
- Perform regular review of client records to assure care appropriateness and documentation quality.
- Manage daily operations using the report library in the appropriate system.
- Meet staff needs (education, training, reinforcements); support staff understanding of nursing standards, regulatory policies, and organization policies.
- Conduct performance improvement activities; measure PI indicators quarterly; complete clinical record reviews, trending reports, infection logs, and incident reports.
- Maintain relationships with referral sources, community, patients, families, and subcontractors; investigate complaints per organization policy.
- Ensure financial accuracy: identify payors, bill within regulatory time frames, obtain authorizations, verify supply billing, maintain PPS matrix report benchmarks.
- Respond to patient/client grievances constructively; ensure customer satisfaction at or above benchmarks.
- Address team member issues/concerns professionally; uphold turnover benchmarks.
- Prepare and conduct team meetings when nurse manager is absent.
- Remain available to clinicians after hours for concerns/questions.
- Perform other duties and/or projects as assigned.
- Adhere to HMH’s organizational and managerial competencies and standards of behavior.
- Lift a minimum of 50 lbs.; push and pull a minimum of 50 lbs.; stand a minimum of 4 hours a day.
- Three (3) to Five (5) years RN experience.
- Proficient in Microsoft Office Suite.
- BSN in progress or to be acquired within three (3) years of hire.
- Two (2) to Three (3) years management experience.
- Homecare experience.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support (HCP) Certification.
The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role. Compensation is determined based on market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding.
