Our nurses spend valuable, meaningful 1:1 time with patients in their homes where they feel most comfortable. Focus on one patient at a time with a flexible schedule and fewer distractions.

Responsibilities
  • Assure compliance with appropriate physician orders and that visit frequency follows orders. Documentation reflects compliance and progress toward discharge goals.
  • Ensure assessment, planning, intervention and evaluation of patient care follow policy, procedure and standard of care. Provide holistic care to patients and significant others. Demonstrate knowledge and competency in administering treatments and procedures according to standards and policies. Utilize an interdisciplinary team and document all activities.
  • Reflect professional standards of care and codes of ethics in clinical judgments and decisions. Document decisions through the EMR.
  • Identify appropriate payors and remain compliant with authorizations. Maintain visit frequency within authorization range using secure messaging to insurance nurses.
  • Ensure assessment, interventions and planning meet organization timelines. Complete medication reconciliation and collaborate with physicians on Plan of Care. Update care plan within two weeks of patient status change.
  • Implement teaching strategies to meet established goals. Document teaching strategies in the chart consistent with goals.
  • Report patient care problems to manager or appropriate nursing management personnel and/or physician in a timely manner with possible solutions.
  • Act as liaison among physicians, manager and other team members to ensure effective coordination and resources. Ensure coordination meets client needs.
  • Communicate pertinent patient information and Plan of Care during transfer of care or when vacation days time‑off are scheduled.
  • Complete routine assignments accurately, consistently and independently on schedule without supervision. Meet productivity requirements.
  • Maintain awareness of new or updated standards and incorporate into practice. Document knowledge of current guidelines.
  • Perform other duties and projects as assigned and demonstrate flexibility.
  • Adhere to HMH organizational competencies and standards of behavior.
  • Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs. and stands a minimum of 8 hours a day.
Qualifications

Education,

Knowledge, Skills and Abilities Required:

  • Associate's Degree in Nursing or Diploma/Certificate in Nursing.
  • Minimum of one (1) or more years RN experience.
  • Ability/Experience inserting IVs.
  • Competency Requirements:
    • With recent (within one year) IV experience:
      Demonstrate inserting peripheral line, accessing ports, PICCs and central lines with a preceptor.
    • Without recent experience:
      Complete a vascular access device (VAD) course and practicum in an outpatient infusion setting; demonstrate insertion under preceptor.
  • ONS/ONCC Chemotherapy/Immunotherapy Certificate within three (3) months of hire. Agency provides paid training.
  • Successful completion of Infusion Test within probationary period with grade 80% or higher.
  • Excellent written and verbal communication skills.
  • Proficient computer skills in Google Suite and/or Microsoft Office.
