Location: Chase Center Rehabilitation and Nursing Services – Logansport, IN
Schedule: Full-Time (35–40 hours/week) & PRN opportunities available
Chase Center Rehabilitation and Nursing Services is seeking a dedicated and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our in‑house rehab team. We can accommodate either discipline for this open position. Whether you’re seeking a stable full‑time role with consistent hours or the flexibility of PRN scheduling, this opportunity allows you to make a meaningful impact in a skilled nursing setting.
We welcome clinicians who bring strong technical skills, professionalism, and a passion for helping residents regain mobility, confidence, and independence.
Responsibilities (PT/PTA)Perform evaluations (PT) and assist with evaluations (PTA) per the scope of practice
Develop or implement individualized treatment plans to improve mobility and functional ability
Deliver evidence‑based therapeutic interventions
Track progress, document outcomes, and update goals as appropriate
Collaborate with the interdisciplinary team to maximize resident outcomes
Educate patients, families, and caregivers on home programs and therapy expectations
Complete documentation accurately and in compliance with all requirements
For PT:
Licensed Physical Therapist (PT) in the state of Indiana
Graduate of an accredited Physical Therapy program
For PTA:
Licensed Physical Therapist Assistant (PTA) in the state of Indiana
Graduate of an accredited PTA program
For Both Disciplines:
Must be at least 18 years of age
SNF/LTC experience preferred
Familiarity with Net Health/Optima is a plus
