• Provide compassionate, relationship‑based care to older adults in an assisted living environment
• Build trust with patients, families, and facility staff through clear communication and consistent presence
• Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient
• Collaborate with the facility’s care team to ensure seamless, coordinated support
• Apply home health or hospice experience to improve outcomes for medically complex patients
• Maintain flexible availability for patient visits and on‑site presence

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non‑probationary)
• Master’s degree from an accredited program
• Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background
• Active DEA license (or eligibility to obtain)
• Availability to work on‑site in Loganville; occasional visits to additional locations may be required
