Requirements

Qualifications:

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary).
  • Master’s degree from an accredited program.
  • Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
  • Active DEA license (or eligibility to obtain).
  • Availability to work on-site in Loganville; occasional visits to additional locations may be required.
