Hospice Chaplain

This range is provided by Advocate Health Care. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range

$26.10/hr - $39.15/hr

Major Responsibilities
  • Provides initial assessment of existing spiritual resources and preferences of patient.
  • Provides spiritual care and emotional support to patients and families.
  • In conformance with the patient's faith and expressed desire, pastoral care services of counseling, sacraments, and support will be available.
  • Responsible for development of a spiritual plan of care reflecting problems, interventions, and outcome goals.
  • Implements the plan of care in conjunction with the hospice interdisciplinary team and according to hospice specific protocols and measures.
  • Serves as liaison for the hospice patient and their local faith community.
  • Facilitates conversations specific to end-of-life planning including goals of care, funeral arrangements, and legacy work.
  • Collaborates with hospice interdisciplinary team with referrals of patients who are in crisis situations.
  • Participates in the QA/CQI process for Advocate Hospice.
  • Responsible for achievement of agency, department and personal goals and objectives.
Licensure, Registration and/or Certification Required

A valid driver’s license issued by the Division of Motor Vehicles.

Education/Experience Required

Master's Degree in Theology, or Master's Degree in Divinity, or Master's Degree in Religious Studies. At least Two (2) units of CPE, accredited by ACPE; NACC units may be considered. 5 units preferred. In good standing with continued endorsement by the appropriate authority of his/her church or religious institute.

Knowledge, Skills & Abilities Required
  • A strong background in functional theology and the ability to apply this in a pastoral manner to individuals and families.
  • Excellent communication skills with knowledge of counseling techniques.
  • Ability to identify client’s needs with consideration to cultural, ethnic, religious, economic, and other diverse backgrounds.
  • Experience and compassion in ministering to persons who are dying and their grieving families.
