Job Title: Occupational Therapist
Location: LaSalle County, IL
Type: Contract, Part-time, PRN, Full-time

Our coordinated, multidisciplinary approach enables us to send a team of professionals delivering the optimal experience and training for each patient’s condition and presentation. Currently covering 13 counties in Illinois:
Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.
Position Overview: Provides occupational therapy services to patients as directed by the Plan of Care in line with the agency’s philosophy, objectives, and policies. Supervises Occupational Therapy Assistants and is responsible for the implementation of OT procedures.
Responsibilities:
- Assist the physician in evaluating patients’ level of function, establishing a treatment plan and goals.
- Perform initial evaluations within 48 hours of referral and re‑evaluations every 30 days, as appropriate, coordinating plan of care with the physician as needed.
- Prepare clinical and progress notes and required summaries, as appropriate.
- Revise plan of treatment and consult the physician for changes in the plan of care.
- Advise and consult with patients’ family and other health‑care staff.
- Coordinate care with agency DON and other staff as appropriate.
- Observe, record, and report any changes in patients’ condition and update the agency on status or schedule changes.
- Supervise the Occupational Therapy Assistant.
- Instruct patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
- Communicate effectively with patients, caregivers, families, and agency staff.
- Provide quality and appropriate care and report issues or concerns to the DON/Administrator.
- Ensure that all tasks are performed in a timely manner.
- Check in on Occupational Therapy Assistants every 4–6 patient visits.
Qualifications:
- Must be a licensed Occupational Therapist in the State of Illinois.
- Must have at least one year of Home Health Care clinical experience.
- Car with required proof of insurance coverage.
- Maintain an annual TB test, CPR certification, and other required certifications.
- Able to read and interpret instructions related to patient care.
- Able to effectively perform and prioritize multiple functions or tasks as needed.
- Must be able to travel to patients’ places of residence.
