Position Overview Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients  homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.

• Prioritize patient health and help prevent hospitalization while providing exceptional care in the home setting.
• Provide skilled nursing care as a Home Health RN Case Manager.
• Proficiency in Well Sky Kinnser EMR for documentation and care coordination.
• Assist with OASIS completion and submission within 24 hours using Andy AI.

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driverficense
• Valid Auto Insurance
• Minimum 1 year home health experience
• Proficiency in Well Sky Kinnser EMR
• Strong clinical assessment and communication skills
