• Provide comprehensive nursing care to patients in their homes
• Conduct clinical assessments and develop individualized care plans
• Coordinate interdisciplinary care and monitor patient progress
• Document nursing care and OASIS data accurately and timely
• Assist patients and families with education on disease management, medication, and self-care
• Support safe discharge planning and transition to community resources
• Maintain compliance with regulatory and agency standards

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driver's License
• Valid Auto Insurance
• Minimum 1 year home health experience
• Proficiency in Well Sky Kinnser EMR
• Strong clinical assessment and communication skills
