$5,000 SIGN ON BONUS for full-time field clinicians. Central Coast Home Health is a fast-growing, locally owned organization serving San Luis Obispo and Santa Barbara counties. We are seeking a full-time or part-time or weekend-only Home Health Physical Therapist in the Northern Santa Barbara area (Lompoc, Santa Ynez, Buellton, Solvang).
Our therapist delivers high quality care to patients in their home environment, collaborating with an interdisciplinary team to address all patient needs, developing relationships, and ensuring continuity of care. This role requires organization and the ability to work independently, managing your own caseload and creating your own work schedule.
Responsibilities- Deliver high quality home health physical therapy services to patients in their homes.
- Collaborate with an interdisciplinary team to address the patient’s needs and ensure continuity of care.
- Develop and maintain therapeutic relationships with patients and families.
- Manage a self-scheduled caseload and organize your work plan to meet patient needs.
- Current California Physical Therapy license.
- CPR certification.
- Reliable vehicle to drive for work and a valid driver’s license with a clean driving record.
- Auto insurance coverage (maintain as required).
- Home Health experience is a plus; new grads are encouraged to apply.

- Compensation by visit: routine visits $69.00-$79.00; evaluations $91.00-$104.00; admissions $172.50-$197.50 per visit. Actual pay rates vary by visit type, experience, location, and seniority.
