Registered Nurse (Lompoc) – Home Health

Omega Home Health Care Services, Inc. invites applicants for a Registered Nurse position in the Lompoc region. The role involves providing comprehensive home health care to patients across many counties in California.

Base pay range: $65.00/hr – $100.00/hr

Per‑visit pay: $65 – $70 per visit (based on experience); $100 SOC Oasis/visit

• Complete an initial assessment of patient and family to determine home care needs and provide a complete physical assessment and history of current and previous illnesses.
• Regularly re‑evaluate patient nursing needs.
• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions; include the patient and the family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician.
• Counsel the patient and family to meet nursing and related needs.
• Provide health‑care instructions to the patient as appropriate per assessment and plan of care.
• Identify discharge‑planning needs as part of the care plan development and implement them before the patient’s discharge.
• Prepare clinical notes and update the agency as needed.
• Communicate with the agency regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required.
• Communicate with community health‑related persons to coordinate the care plan.
• Ensure that arrangements for equipment and other necessary items and services are available.
• Instruct, supervise, and evaluate home‑health aide care provided every two weeks.

• Registered nurse with current licensure to practice professional nursing in California.
• Graduate of a National League for Nursing accredited nursing school.
• Maintains a current CPR certification.
• Licensed driver with an insured automobile that meets state/or organization requirements and is in good working order.
• Ability to perform lab draws, wound care, and IV procedures (PICC line, central, midline, etc.).
• Experience in tracheostomy management.
• Minimum of two years of experience, at least one of which is in home health nursing.
• Self‑directed and able to work with minimal supervision.
• Excellent observation, problem‑solving, verbal, and written communication skills; nursing skills per competency checklist.
• Ability to organize and prioritize workload independently.
• Experience supervising home health aides (management experience not required).

Employee must perform prolonged or considerable walking or standing, and occasionally lift or move a minimum of 50 pounds. Must be able to transfer patients, lift supplies and equipment, and perform extensive reaching, stooping, bending, kneeling, or crouching. Adequate visual acuity and hearing are required.
