** The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.

* What you’ll do:
*** Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
* Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
* Perform the initial personal care services for new patients.
* Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
* Inform patients and families of Para Med policies and scheduling practices, as well as the expected time of care.
* Maintain current and accurate patient records.
* Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
* Stay abreast of changes in practice setting that affect scope of practice.

* What you bring:
*** Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
* Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
* Knowledgeable in best practice standards for community health care and services.
* Strong oral and written communication skills
* Excellent organizational skills
* Demonstrated competency in patient centered care principles and practices.
* Knowledgeable of community resources available for patients and families.
* Strong independent critical thinking and problem-solving skills.
* Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
* Access to a personal vehicle required##
