- Supervise and support PSW’s and HSW’s to ensure practices comply with all legislated standards and company policies
- Respond to and advise/resolve concerns from community staff, patients/families, and other team members
- Provide ongoing leadership through regular huddles, face‑to‑face meetings, and one‑on‑ones
- Initiate patient safety incident investigations and document actions to meet timelines and action‑plan requirements
- Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines
- Engage patients in problem‑solving using a collaborative approach, offering health teaching and building rapport and trust to identify patient health‑care needs, strengths, capacities, and goals
- Engage staff in problem‑solving, offering education, coaching, and mentoring in resolution of problems
- Maintain knowledge, skills, and attitudes to provide safe, efficient, and effective service in all practice areas by staying abreast of changes in practice settings that affect scope of practice
- Current RPN registration with the College of Nurses of Ontario and entitlement to practice without restrictions
- Minimum of 5 years’ experience as a practicing Registered Practical Nurse
- Comprehensive patient health assessment skills are required
- Understanding of the principles of community nursing practice
- Proven experience leading and managing a team
- Knowledge of clinical documentation standards and practice guidelines is required
- Excellent organizational, interpersonal, and teaching skills
- Commitment to professional development in clinical practice relevant to home‑health care
- Effective critical‑thinking and problem‑solving skills
- Valid driver’s license and access to reliable transportation
- Current, clear Vulnerable Sector Screening (VSS) certificate
- Certification as a Community Health Nurse (CCHN(C)) is considered an asset
