Job Description:

The Nurse Practitioner (Locum) works with addiction counsellors, nurses, primary care providers and other community health service providers in offering addiction medical support for individual clients living with mental illness and addiction through a harm-reduction lens. You will work interdependently with staff members to provide shared, client-centered care that emphasizes health promotion/protection, disease prevention and client education, while providing episodic primary health care to CMHA TVAMHS clients who do not have a primary care provider and consults with collaborative physicians for clients who have unstable/complex health conditions.

How You Will Be Making a Difference
  • Maintain requirements for practice, including a license for a Registered Nurse in the Extended Class, NP/Physician Collaborative Practice Agreement, and malpractice insurance.
  • Maintain confidentiality and act in an ethical manner, consistent with skilled expectations and the College of Nurses of Ontario (CNO).
  • Retain clinical competence through relevant evidence‑based research and legislation.
CWSP / Outreach & Community Partnerships & Withdrawal Management
  • Work in partnership with Community Withdrawal Support Program (CWSP), community agencies and programs and Withdrawal Management
  • Integrate CMHA TVAMHS counseling services and support for clients.
  • Coordinate service provision with other health care partners in the development of community care plans and wrap‑around services.
  • Provide inter‑discipline home visits with vulnerable clients or with clients with transportation and other barriers to care services when required.
  • Assist in program development specific to CWSP that align with best practice standards, evidence‑based research and CMHA TVAMHS program mandate.
  • Promote the integration of addiction-based medicine and primary care.
  • Offer educational opportunities and consultation as appropriate.
What You’ll Need to Apply
  • Police Vulnerable Sector Check completed in the last six months.
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (proof of registration required).
  • Successful completion of the Prescribing Narcotics and Controlled Substances course (copy of certification required).
  • Current certificate in cardiopulmonary resuscitation (CPR).
  • Valid Driver’s Licence and access to a vehicle with adequate insurance to carry out job duties.
Preferred Qualifications
  • Proficiency with computers, smartphones, variety of software programs including presentation technology.
  • Familiarity with Zoom, Microsoft Teams and other video conferencing technology.
  • Master's Degree.
  • Suboxone online education program.
  • Minimum of two to three years of community health care experience.
  • Experience in addiction medicine and harm reduction.
  • Excellent clinical, interpersonal and organizational skills.
  • Project planning and evaluation skills would be an asset.
