Role Description

This community‑based role for a Registered Practical Nurse (RPN) located in London, ON, involves providing care and support to senior clients, developing and implementing individualized care plans, administering medications, monitoring health conditions, and supporting daily living activities. The RPN collaborates with families, caregivers, and other healthcare professionals to ensure clients’ well‑being and address any changes in their condition promptly and effectively, contributing to a safe, compassionate, and respectful environment for all clients.

Qualifications
  • Current RPN license and knowledge of nursing practices and regulations in Ontario
  • Proficiency in patient care, health assessments, and individualized care planning
  • Competency in medication administration, wound care, and monitoring of health conditions
  • Effective communication, collaboration, and documentation skills
  • Compassionate and patient‑focused with a strong commitment to providing high‑quality care
  • Experience in senior care, including dementia, Alzheimer’s, or chronic illness management, is an asset
  • CPR/First Aid certification and compliance with all regulatory health and safety requirements
  • Proficiency in maintaining confidentiality and adhering to professional standards
