We are expanding our Primary Care Action Teams and seeking a Nurse Practitioner (MRP) with a commitment to patient education, health promotion, and disease prevention. This role emphasizes providing primary care as a member of a collaborative team serving over 169,000 patients in Ontario. Your expertise will shape a new model of care in the community.

Key Responsibilities:

• Serve as the primary care provider to an individual panel of patients

• Provide accessible primary health care services

• Emphasize health promotion and disease prevention

• Collaborate with the broader primary care team

• Ensure follow-up of critical lab results after hours

Requirements:

• Registration with the College of Nurses of Ontario

• 2 years of Nurse Practitioner experience preferred

• Master of Science in Nursing preferred

• Experience in Community/Primary Care settings

• Reliable transportation for travel between sites
