Position: Full-Time Registered Nurse at TVFHT
Become a vital part of TVFHT as a Full-Time Registered Nurse in London, Ontario. Deliver patient-centered care while collaborating with a multidisciplinary health team across varied community sites.

TVFHT, one of Ontario’s largest family health teams, is expanding its Primary Care Action Teams. As a Registered Nurse, you will utilize your clinical assessment skills and knowledge of Chronic Disease Management to enhance patient care. Your commitment to education, both individually and in groups, will help our patients achieve health equity and improved outcomes.

Key Responsibilities:

• Collaborate with team members on patient care initiatives

• Conduct comprehensive patient assessments and implement medical directives

• Educate patients through tailored programming

• Utilize EMRs and virtual care technologies effectively

• Participate in system integration and health innovations

Requirements:

• Active Registered Nurse (RN) status with CNO

• Bachelor of Science in Nursing (BScN) preferred

• Experience in patient education

• Valid driver’s license and vehicle access

• Ability to thrive in fast-paced environments
