In this critical role, you will perform client-centered care while collaborating with addiction counsellors, nurses, and community health partners. Additionally, you'll ensure comprehensive care by consulting with physicians and addressing complex client conditions through effective community service integration.
Key Responsibilities:
• Sustain RN licence and meet all practice obligations
• Provide supportive health care focusing on addiction
• Collaborate with CWSP and local health agencies
• Conduct home visits for clients with limited access
• Promote program development in alignment with research
Requirements:
• Registered Nurse (Extended Class) in Ontario
• At least 2–3 years of experience in community care
• Valid driver’s licence and recent police check
• Current CPR certification and dedication to client care
• Proficiency with communication technologies
