In this rewarding position, you will serve as the Most Responsible Provider within an interprofessional team. Your work will facilitate disease prevention, enhance patient outcomes, and ensure accessible healthcare across diverse communities. Additionally, you'll be responsible for critical lab results and after-hours patient follow-ups to support comprehensive care.

Key Responsibilities:

• Provide comprehensive health care as the Most Responsible Provider

• Promote health and deliver disease prevention services

• Collaborate with diverse healthcare professionals

• Manage critical lab results and after-hours follow-ups

• Keep accurate patient records using Electronic Medical Records

Requirements:

• Must be registered with the College of Nurses of Ontario

• At least 2 years of Nurse Practitioner experience preferred

• Baccalaureate of Science in Nursing required

• Certification in Primary Health Care Nurse Practitioner Program

• Excellent communication and interpersonal skills
