Position: Nurse Practitioner Transformation Lead (Full-Time, Regular)

The full-time Nurse Practitioner Transformation Lead will play an integral role in advancing TVFHT’s commitment to increasing attachment to teams through Nurse Practitioner as Most Responsible Provider (NP‑MRP). This impactful transformation will improve service access through leading NPs to shift to a formal MRP role & support NPs and teams to achieve full scope. It will involve support of attachment work and developing metrics around complexity, panel distribution, NP‑MRP models and model outcomes.

The role will also support future key service NP initiatives to offer impactful services to improve access and health outcomes of the communities we serve. It is expected that this role will guide clinical work and NP MRP transformation at the site level.

This position will serve London, Middlesex, Elgin and Oxford communities in alignment with Provincial scope and opportunities.

Qualifications
  • Current registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario.
  • Master of Nursing (MN) required, Doctor of Nursing Practice (DNP) an asset.
  • Experience in primary care as formal MRP required, with a demonstrated record of leadership in clinical practice change or QI.
  • Understanding of quality improvement methodologies, implementation science, or applied health research preferred.
  • Proven ability to lead interdisciplinary teams, mentor, and drive evidence‑informed practice improvements.
  • Excellent communication skills: both written and verbal.
  • Level of comfort or willingness to become familiar with virtual meeting technology.
Duties & Responsibilities
  • Direct NP MRP practice transformation through change management principles and quality improvement frameworks.
  • Lead the integration of clinical guidelines, decision support tools and evidence‑based best practices into organizational care delivery frameworks, overseeing NP‑MRPs in their application and evaluating outcomes at the program level.
  • Provide formal clinical oversight to NP‑MRPs in areas of advanced practice, evidence-based practice and clinical decision‑making, including performance assessment and accountability for practice standards.
  • Supervise NPs transitioning to NP‑MRP positions, including newly hired NP‑MRPs.
  • Develop and evaluate innovative care models and workflows to improve access, continuity, and interdisciplinary collaboration.
  • Participate in program evaluation in collaboration with leadership and the Quality team. Develop practice-level metrics to assess impact of interventions and inform iterative improvements.
  • Support health equity and population health by embedding equity‑informed approaches into clinical transformation strategies.
  • Disseminate outcomes through internal reports, professional presentations, or peer‑reviewed publications.
  • Participate in provincial, regional and academic partnerships to support knowledge exchange and build capacity.
  • Monitor policy developments and emerging trends in healthcare that impact NP scope in primary care.
  • Contribute to the development of organizational responses to relevant legislative or regulatory changes, supporting the integration of NP‑led models of care.
  • Foster connections with professional associations (e.g., NPAO, RNAO) and contribute to position statements or submissions that align with the TVFHT’s strategic goals.
  • Represent the FHT and the NP profession in regional or provincial forums, working groups, or stakeholder consultations to advocate for policies that advance access, primary care innovation, and full NP MRP scope of practice.
  • Collaborate with community partners and internal teams to address social determinants of health, improve access for underserved populations, and support both local and system-level gaps in care delivery. This includes identifying priority populations, advocating for equitable resource allocation, and co‑de‑developing strategies that bridge service fragmentation across the continuum of care.
  • Support the recruitment and selection of NP‑MRPs through candidate interviews, assessments, and hiring recommendations.
  • Travel to partner sites or external stakeholder meetings may be required.
