Transformation Lead Nurse Practitioner

Lead the Nurse Practitioner transformation at Thames Valley Family Health Team, enhancing patient access and team integration. This full-time role focuses on advancing Nurse Practitioner as Most Responsible Provider.

In this impactful position, you will drive the shift towards NP-MRP models across London, Middlesex, Elgin, and Oxford communities. The role demands strong primary care experience and a passion for improving health outcomes through evidence-based practice and quality improvement methodologies. You will lead clinical work, support NP transitions, and develop metrics for better service delivery.

Key Responsibilities:

• Direct NP MRP practice transformation using change management

• Lead integration of clinical guidelines into care delivery

• Supervise NPs transitioning to NP-MRP roles

• Collaborate on program evaluation with leadership

• Disseminate outcomes through various professional channels

Requirements:

• Current Ontario Nurse Practitioner registration

• Master of Nursing required, DNP an asset

• Experience as formal MRP in primary care

• Proven leadership in clinical practice change

• Excellent communication skills, both written and verbal
