Thames Valley Family Health Team is expanding its Primary Care Action Teams and seeking a Full-Time Registered Nurse. Ideal candidates should possess a Bachelor of Science in Nursing and have strong clinical assessment skills. This role emphasizes individual and group patient education in a collaborative, dynamic setting involving diverse healthcare professionals.
Key Responsibilities:
• Provide comprehensive patient-centered care within the interdisciplinary team
• Conduct individual and group patient education sessions
• Implement medical directives with full scope practice
• Utilize EMRs and virtual care technologies effectively
• Collaborate with community resources for enhanced patient management
Requirements:
• Registered Nurse (RN) in good standing with CNO
• Bachelor of Science in Nursing (BScN) preferred
• Experience in chronic disease management models
• Valid driver's license and access to a vehicle
• Strong interpersonal and communication skills
