Take on a pivotal role as a Nurse Practitioner with Thames Valley Family Health Team in Elgin County. This position allows you to provide key patient care and health education.
We are expanding our Primary Care Action Teams and seeking a dedicated Nurse Practitioner. You will be the primary care provider for your own panel of patients, emphasizing health promotion. Collaborating with a wider healthcare team, you will significantly contribute to accessible healthcare for various demographics.
Key Responsibilities:
• Provide comprehensive care as Most Responsible Provider
• Deliver health promotion and disease prevention services
• Collaborate with interprofessional healthcare teams
• Manage critical value lab results and after-hours follow-ups
• Maintain accurate patient records using EMR
Requirements:
• Current registration with the College of Nurses of Ontario
• 2 years of practice as a Nurse Practitioner preferred
• Baccalaureate of Science in Nursing required
• Certification in Primary Health Care Nurse Practitioner Program
• Excellent communication and interpersonal skills
