As part of Ontario’s leading family health teams, you’ll work alongside physicians, dietitians, and social workers, focusing on comprehensive, patient-centered care. The successful candidate will apply their nursing skills, enhance Chronic Disease Management understanding, and engage in both individual and group patient education.

Key Responsibilities:

• Assess patient needs and implement medical directives

• Provide individual and group education to patients

• Collaborate across disciplines for integrated care

• Utilize EMRs and virtual tools for assessments

• Contribute to health system innovation and equity

Requirements:

• Registered Nurse (RN) in good standing with CNO

• Preferred Bachelor's degree in Nursing

• Strong clinical assessment and education experience

• Valid driver’s license and regular vehicle access

• Comfortable working in a team-based model
