As a Nurse Practitioner with CMHA TVAMHS, you'll collaborate with addiction counselors and primary care providers. Your focus will be on enhancing the health of clients facing mental illness and addictions, providing primary healthcare, and ensuring ethical and confidential practices. This part-time position offers flexible shifts catering to community needs.
Key Responsibilities:
• Offer psychological and medical support for addiction recovery
• Coordinate care with other healthcare providers
• Execute individualized community care plans
• Conduct home visits for hard-to-reach clients
• Support the integration of service delivery models
Requirements:
• Maintain current RN Extended Class registration
• Recent Police Vulnerable Sector Check needed
• Active CPR certification
• Completion of narcotics prescribing course required
• Driver’s License and accessible vehicle mandatory
