As a Nurse Practitioner (Most Responsible Provider), you will play a vital role in healthcare delivery while working alongside physicians and other professionals. This position involves managing an individual patient panel and actively participating in ongoing quality improvement projects and patient education strategies.

Key Responsibilities:

• Provide primary care that aligns with team governance and policies

• Manage chronic diseases and provide health promotion

• Enhance patient access to healthcare and treatments

• Collaborate for effective interdisciplinary care coordination

• Oversee development and evaluation of quality management initiatives
Requirements:

• Current registration with Ontario's College of Nurses

• Master's degree in Primary Care Nurse Practitioner Program

• At least 2 years of experience in a primary care setting

• Strong leadership and communication skills

• Experience with EMR systems like TELUS PS Suites
