Key Responsibilities:
• Perform detailed clinical assessments for diverse patient needs
• Deliver individualized and group health education
• Coordinate with interdisciplinary teams for holistic care
• Leverage EMRs and virtual care solutions
• Implement chronic disease management strategies
Requirements:
• Valid Registered Nurse (RN) status with CNO
• Bachelor of Science in Nursing (BScN) preferred
• Experience in patient education and chronic disease management
• Ability to use a vehicle with a valid driver's license
• Comfortable in fast-paced, team-centric settings
