Nurse Practitioner; MRP

Position: Nurse Practitioner (MRP) Role
Thames Valley Family Health Team is searching for a Nurse Practitioner (MRP) to expand its patient services in London, Ontario. Deliver essential primary care and support a diverse patient community.

The Nurse Practitioner will act as the Most Responsible Practitioner and serve an individual patient panel, combining health promotion and education within a collaborative primary care environment. Applicants should possess a strong clinical background and two years of relevant experience. This role is fully onsite at our location on Wonderland Rd. South, offering essential health care services.

Key Responsibilities:

• Manage care for a defined patient panel

• Provide health education and disease prevention strategies

• Conduct follow-ups on lab results as necessary

• Work as part of a primary healthcare team

• Use EMR systems for documentation and patient tracking

Requirements:

• Valid registration with College of Nurses of Ontario

• Preferred two years in primary care practice

• Required BSc in Nursing; MSc preferred

• Active membership in RNAO or NPAO

• Community care experience is essential
