In this collaborative role, you will assess and manage both chronic and acute health conditions while leading clinical initiatives in long-term care. Your expertise will contribute to care planning, palliative support, and engaging families about their loved ones’ needs. Participate in resident conferences to ensure comprehensive, person-centered care.
Key Responsibilities:
• Evaluate and diagnose resident health conditions
• Work with healthcare providers for effective care strategies
• Lead initiatives focused on resident care needs
• Support families throughout treatment processes
• Participate in identifying care trends for educational development
Requirements:
• Minimum one year in geriatric or long-term care nursing
• Registered as a nurse in the Extended Class in Ontario
• Knowledge of Long Term Care Act and related regulations
• Leadership knowledge or experience an asset
• Certifications in relevant specialties are a plus
Utilize your nursing skills to enhance care quality and support residents at Age Care.
