Position: Full-Time Nurse Practitioner Palliative Care
Location: Southwestern Ontario

Become a vital part of Ontario Health atHome's Palliative Care Consultation Team. This permanent opportunity requires expertise in diagnosing, treating, and managing palliative symptoms and chronic diseases.

Your role will include working closely with community patients who need specialized care, ensuring high standards of quality and safety throughout.

Key Responsibilities:

• Provide assessments for palliative symptom management

• Collaborate with Hospice for effective patient care

• Diagnose and manage complex pain and symptoms

• Travel between various Ontario Health atHome sites

• Ensure adherence to professional standards and guidelines

Requirements:

• Current Nurse Practitioner registration with the College of Nurses

• Specialized education in palliative care management

• Knowledge of community health care resources

• Access to a vehicle and valid Ontario Driver’s License

• Proficient in using computer systems
