The Health Centre is seeking a dynamic Primary Care Nurse Practitioner to join our clinical team. This position offers a unique opportunity to deliver health care to those facing barriers such as poverty and mental health issues. You will collaborate with interdisciplinary team members to devise effective care plans that prioritize health equity.

Key Responsibilities:

• Provide health care in a refugee health clinic setting

• Liaise with interdisciplinary team members for cohesive care

• Address health issues related to trauma and social determinants

• Facilitate health promotion and education initiatives

• Support newcomers and marginalized individuals' health journeys

Requirements:

• Master or Bachelor of Science in Nursing

• Active Primary Health Care Nurse Practitioner Certificate

• 3-5 years of community primary care experience

• Knowledge in trauma-informed care

• Commitment to health equity and inclusivity
