This role is a key position within one of Ontario's largest family health teams, dedicated to providing essential services for more than 169,000 patients. As a Nurse Practitioner, you will focus on holistic health management while supporting families in the community. Candidates should showcase their commitment to health promotion and disease prevention.

Key Responsibilities:

• Deliver primary health care services to individual patients

• Engage in health promotion and patient education initiatives

• Be part of the broader healthcare team for comprehensive care

• Manage after-hours critical lab results effectively

• Utilize EMR and other technologies for patient communications

Requirements:

• Nurse Practitioner registration in Ontario

• Minimum of two years of relevant experience preferred

• Completion of Primary Health Care NP Program

• Bachelor's degree in Nursing required

• Active membership with RNAO or NPAO
