Join Ontario Health atHome as a Nurse Practitioner focused on Palliative Care. This rewarding full-time position is based in the Erie St. Clair Region, offering a chance to support vital patient care.
As a Nurse Practitioner, you'll work with the Palliative Care Consultation Team, providing essential services to patients with complex pain and symptoms. This role emphasizes community integration and aims to prevent institutionalization. You'll play a key role in ensuring a patient-centred approach while navigating the intricacies of community healthcare resources.
Key Responsibilities:
• Deliver direct consultations for palliative patients
• Evaluate and manage chronic disease symptoms
• Collaborate effectively with Hospice and other healthcare professionals
• Travel as needed between Ontario Health sites
• Maintain best practices according to healthcare policies
Requirements:
• Registration as a Nurse Practitioner with the College of Nurses
• Specialized education in palliative care and symptom management
• Good understanding of community healthcare legislation
• Access to a vehicle and valid Ontario Driver’s License
• Ability to operate computer systems
