In this role, you'll provide crucial wound care and treatment for residents. Collaborate with physicians and the healthcare team to design personalized care plans and monitor residents' skin integrity. Key Responsibilities:
• Monitor and treat wounds and skin conditions effectively
• Collaborate on treatment plans with interdisciplinary teams
• Ensure accurate documentation of assessments
• Recommend care plan changes based on monitoring
• Educate residents and their families on wound care
Requirements:
• Active California LVN license required
• Certification in Wound Care necessary
• At least two years of nursing experience
• Proficiency in assessments and documentation skills
• Caring, compassionate approach towards patient care
