Location: Southwestern Ontario
Join Advantmed as a Nurse Practitioner for in-home wellness assessments. Enjoy flexible weekdays with strong support, earning ~$100 per visit, plus mileage.
This PRN role targets licensed Nurse Practitioners who seek consistent income through structured assessments. The position involves comprehensive in-home evaluations for patients with various healthcare needs, requiring professional autonomy and accountability. Enjoy operational support while maximizing visit frequency.
Key Responsibilities:
• Conduct Annual Wellness Visits and chronic assessments
• Perform focused histories, exams, and medication reviews
• Screen for key health indicators like Hemoglobin A1C
• Educate patients on preventive care and chronic management
• Document assessments in the EMR for risk adjustment
Requirements:
• Active NP license in applicable state
• 3+ years patient care experience is preferred
• Proficient with EMR systems and independent work
• Experience in risk adjustment or in-home visits
• Willingness to obtain additional licensure if necessary
