Position: Nurse Practitioner Needed for In-Home Care
Location: Southwestern Ontario

Become a valued Nurse Practitioner with Advantmed, focusing on in-home assessments. With flexible weekdays and ~$100 per visit, this is a supportive PRN role.

Advantmed is in search of dependable Nurse Practitioners who can perform structured wellness visits in patients’ homes. This position allows NPs to manage their schedules while ensuring consistency and quality care through strong operational support. Candidates should be ready to engage in direct patient care with ongoing availability.

Key Responsibilities:

• Perform in-home wellness and risk assessments

• Complete focused exams and medication reviews

• Administer screenings like Diabetic Retinal Exams

• Educate patients about chronic disease management

• Record visits accurately in EMR systems

Requirements:

• Valid NP license in the state of practice

• Minimum 3 years of patient care preferred

• Comfortability with EMR and independent practice

• Experience in home care or risk assessments

• Open to additional licensure requirements
