As a key member of the Thames Valley Family Health Team, you will engage with individuals facing barriers to healthcare access, including those impacted by poverty, mental health issues, and housing instability. You will utilize social justice principles and provide trauma-informed care while enhancing your nursing skills in a dynamic community environment.
Key Responsibilities:
• Provide front-line outreach and nursing care
• Connect clients to internal and external referrals
• Assess and address health needs within marginalized communities
• Document care using Electronic Medical Records
• Collaborate with interprofessional health teams
Requirements:
• BScN or equivalent nursing degree
• Current registration with the College of Nurses of Ontario
• Minimum 2 years of nursing experience
• Knowledge of social determinants of health
• Strong communication and interpersonal skills
