SE Health seeks a dedicated practitioner nurse to complete comprehensive assessments and develop tailored care plans while collaborating with community services. Working 2-3 days per week, you will take the lead in client care and ensure seamless transitions. Key Responsibilities:
• Conduct comprehensive client assessments and create care plans
• Lead client care as point of contact within the care team
• Collaborate with community support services for client transitions
Requirements:
• Certificate of Competence from the College of Nurses of Ontario
• Availability for 2-3 days/week and on-call duty
• Updated immunization and clear criminal background check
• Current CPR certification required
• Physically able to perform demanding tasks
