As part of SE Health, your responsibilities include comprehensive assessments and care plan development tailored to clients' specific needs. You'll work collaboratively with healthcare teams and community services to ensure clients receive optimal care and support. SE Health prioritizes your professional growth, providing numerous opportunities for education and mentorship.
Key Responsibilities:
• Complete client assessments and care plan implementation
• Take lead responsibility for assigned client caseloads
• Support client transitions by connecting with community services
Requirements:
• Current Certificate of Competence with College of Nurses of Ontario
• Availability for 2-3 days per week and in the on-call rotation
• Updated immunization and clear background checks required
• Willingness to attain necessary certifications
• Current CPR certificate needed
