*** As a Licensed Practical Nurse, you will coordinate patient care in sunny Pennsylvania while focusing on transitional and chronic disease management. Support physicians and improve patient outcomes effectively.

This role requires valid LPN licensure in Pennsylvania alongside a high school diploma. Engage in call center duties and collaborate with various healthcare providers to ensure seamless care.

Experience with third-party payers like Medicare is highly valued, as is proficiency with Microsoft applications for effective communication.

Key Responsibilities:

• Perform call center functions to support patient care

• Assist physicians with patient care development

• Coordinate with outside healthcare providers efficiently

• Serve as a liaison among various healthcare professionals

• Monitor effectiveness of care plans for patients

Requirements:

• Valid LPN licensure in the state of Pennsylvania

• High school diploma or equivalent

• CPR certification required within the first year

• Preferred two years of office experience

• Knowledge in third-party regulatory requirements preferred
