In this regular part-time position, you will provide holistic nursing care within a multidisciplinary team dedicated to addressing the psychological needs of veterans. Your responsibilities include assessing mental health conditions such as PTSD, conducting treatment, and coordinating care with community partners, ensuring a supportive environment for clients and their families.
Key Responsibilities:
• Assess and document patients' mental health progress
• Coordinate with external providers for comprehensive care
• Facilitate family involvement in treatment plans
• Teach and support clients and families about their conditions
• Engage in research and teaching within the healthcare community
Requirements:
• Active registration with the College of Nurses of Ontario
• Valid CPR certificate and experience in outpatient mental health
• Knowledge of the Mental Health Act and client care coordination
• Excellent communication, organizational, and critical thinking skills
• Willingness to travel to other clinic sites as required
