Key Responsibilities:
• Maintain effective Care Plans with Ontario Health atHome
• Provide services both in-person and virtually
• Collaborate with interdisciplinary care teams
• Care for patients in diverse environments
• Carry out other clinical roles as required
Requirements:
• RN or RPN degree or diploma
• Registration with CNO in good standing
• Strong commitment to patient care
• Valid driver’s license and vehicle
• Current CPR Certification and immunization records
