Key Responsibilities:
• Provide primary health care and preventative services
• Conduct thorough patient assessments and care management
• Order and evaluate diagnostic tests for patient needs
• Develop and implement personalized care strategies
• Collaborate with intersect oral health partners for service improvements
Requirements:
• Master of Nursing - Nurse Practitioner (MN-NP)
• Proven experience in health promotion and community service
• Strong skills in pharmacotherapeutics and patient advocacy
• Ability to facilitate case management for at-risk demographics
• Passion for enhancing prenatal support initiatives
