Salary Range: $41.15 - $58.98 /hour

The Registered Nurse with the Geriatric Ambulatory Access Team will be part of an exciting team that supports patients/family caregivers in accessing Community Geriatric Services through assessment and triage by telephone assessment in an office environment.

• Valid registration with the College of Nurses of Ontario
• Valid CPR certificate
• Previous experience with assessments in an inpatient or community-based geriatric population
• Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
• Ability to function as an independent practitioner and a highly effective team member
• Knowledge of how to mobilize community health resources and systems
• Ability to synthesis clinical information succinctly and accurately both written and verbally
• Exceptional verbal and written communication skills with a variety of community and organizational partners
• Demonstrated application of case management skills in complex clinical situations
• Superior interpersonal and relationship building skills
• Ability to be flexible in the scheduling of work hours
• Exceptional computer skills
• Exceptional conflict management skills
• Demonstrated ability to use best practice and research
• Strong ability to think critically around clinical issues
• Strong ability to communicate/transfer clinical summaries successfully
• Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

• Knowledge of registration and scheduling
