Elevate resident care standards as a Nurse Practitioner with Age Care London. This full-time role emphasizes collaboration with medical teams to diagnose and manage patient health effectively.
In this regular full-time position, the Nurse Practitioner will play a crucial role in delivering quality care, assessing resident conditions, and liaising with interdisciplinary teams. You will be pivotal in advocating for residents’ interests while participating in care conferences and providing clinical leadership to our long-term care team.
Key Responsibilities:
• Collaborate with the medical team for resident care
• Assess and diagnose acute and chronic health issues
• Participate in care conferences for improved resident outcomes
• Provide leadership in palliative and end-of-life care
• Analyze trends to enhance nursing practice and education
Requirements:
• Minimum one year experience in geriatric nursing
• Current registration with the College of Nurses of Ontario
• Knowledge of Long Term Care Act and related legislation
• Certifications in gerontology or palliative care are assets
• Leadership experience in healthcare considered advantageous
