St. Joseph's Family Medical Centre is looking for an experienced Registered Practical Nurse to join their team. This part-time role will involve providing comprehensive nursing support, including assessments and education for patients with complex health challenges. Key Responsibilities:
• Provide nursing assessments and supportive care
• Educate patients about chronic disease management

Requirements:
• Valid registration with the College of Nurses of Ontario
• Minimum of 2 years experience in primary care
