As a Nurse Practitioner at the Village of Glendale Crossing, you will be an essential part of delivering high-quality clinical care. The role involves conducting comprehensive geriatric assessments and leading a team dedicated to quality improvement. Key Responsibilities:
• Lead multi-disciplinary care teams effectively
• Assist with geriatric assessments
• Develop and oversee palliative care initiatives
• Conduct and communicate diagnostic testing
• Engage in quality improvement projects
Requirements:
• Valid Nurse Practitioner registration in Ontario
• Experience with comprehensive assessments
• Current CPR and AIS certifications
• Familiarity with long-term care policies
• Positive attitude and strong leadership skills
