Long-Term Care Nurse Practitioner

South Centennial Manor seeks a Nurse Practitioner with advanced clinical judgment and geriatric expertise for autonomous long-term care practice.

This Nurse Practitioner provides resident-centered medical care within a collaborative interdisciplinary environment. Reporting to the Director of Care, the NP works with the Medical Director and attending physicians. The position supports evidence-based practice, aging-in-place initiatives, and system integration across care sectors.

• Assess, diagnose, treat, and medically manage residents independently
• Provide consistent on-site clinical leadership for residents
• Strengthen continuity of care across long-term care services
• Reduce avoidable hospital transfers through hospital avoidance strategies
• Support complex geriatric needs, including dementia and responsive behaviours

• Master’s Degree in Nursing or BScN with NP certification
• Current Ontario Extended Class registration with the College of Nurses
• Five years RN experience and two years NP experience
• BCLS/ACLS certification; gerontology or palliative care preferred
• Electronic medical record and Microsoft Office proficiency
