We believe in a support‑first approach for those who come into contact with our justice system as a result of addiction or mental health concerns. As Transitional Case Manager, Justice, you’ll work with clients to address the underlying issues that led to contact with the justice system, to provide counselling that builds on their strengths, and helps them make steps towards recovery with an aim to improve their quality of life, reduce harm, and prevent future interactions with the justice system.
You’ll also be responsible for attending court and communicating with justice partners regarding progress and to advocate for the best interests of your clients.
Starting rate for this role is $27.14 per hour (Level1 – Direct Service), with a wage differential, based on regulation: $30.01 per hour for Level1 – Direct Service, Regulated.
Successful candidates who are registered with one of the designated professional colleges (OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation. Successful external applicants will start at Level1, with grid progression based on seniority up to Level8 ($34.41 per hour for non‑regulated, and $37.26 per hour for regulated). For internal applicants, grid level placement will occur as per the Collective Agreement.
- 70 hours over two‑week period; 8:30am to 4:30pm, Monday to Friday, with one unpaid hour for lunch. These hours may vary on occasion.
- 9% in lieu of benefits.
- Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP).
- Perform intake at probation meetings, parole meetings, or on site at jails in Southwestern Ontario.
- Attend court sessions, as needed, and provide court support to individuals dealing with current or past charges.
- Liaise with legal professionals (e.g. lawyers, probation officers, social workers).
- Liaise with court system, detention centres, community‑based agencies in order to provide support and/or discharge planning upon individual’s release from custody.
- Demonstrated ability to work with individuals living with multiple challenges related to involvement with the criminal justice system, mental illness, substance use, complex health challenges, street‑level sex work, poverty, homelessness, abuse and trauma.
- Assist with short‑term crisis management by providing an urgent/emergent response to individuals in need of case management supports.
- Engage with clients on a short‑term basis to support their immediate needs.
- In collaboration with the client, conduct a variety of assessments (skills, functional ability, needs, barriers, etc.).
- Assess the adequacy of resources/supports currently available to the client.
- Determine the impact of psychiatric symptoms on functioning.
- Identify risk factors related to client and staff safety as well as relapse.
- Assess barriers to successful community living (e.g. treatment options, transportation, finances, etc.).
- Obtain information, as needed, from other relevant sources.
- Ensure the assessment incorporates cultural and ethnic factors.
- Must have a Police Vulnerable Sector Check (PVSC) completed within the last six months.
- Applicants in need of a PVSC are encouraged to apply for one as soon as possible to avoid delays.
